What It Is
SS-31, also known by its developmental names Elamipretide, MTP-131, and Bendavia, is a synthetic aromatic-cationic tetrapeptide belonging to the Szeto-Schiller (SS) peptide family. It is classified in the research literature as a mitochondria-targeted peptide (MTP) due to its ability to selectively concentrate at the inner mitochondrial membrane independent of mitochondrial membrane potential Szeto, British Journal of Pharmacology, 2014. Structurally, SS-31 alternates cationic and aromatic residues, a motif that researchers have shown allows the peptide to partition into lipid bilayers and interact selectively with cardiolipin, a phospholipid unique to the inner mitochondrial membrane Alexandrescu et al., eLife, 2022.
Summary of Published Research Findings
The foundational discovery work on SS-31/cardiolipin interactions was reported by Szeto and Birk, who described SS-31 as the “first-in-class cardiolipin-protective compound” studied for restoring mitochondrial bioenergetics in various disease and injury models Szeto, Br J Pharmacol, 2014 and Birk & Szeto, Clin Pharmacol Ther, 2014.
In cardiac ischemia-reperfusion research, Brown et al. reported that elamipretide mitigated fragmentation of mitochondrial cristae networks in a rat cardiac ischemia-reperfusion model, using permeabilized ventricular fiber respirometry to characterize mitochondrial structure-function Brown et al., Communications Biology, 2020.
Aging-focused rodent studies have examined SS-31’s effects on skeletal muscle and cardiac function. Whitson et al. reported that elamipretide improved ADP sensitivity in aged mouse mitochondria by increasing substrate uptake through the adenine nucleotide translocator (ANT) Whitson et al., GeroScience, 2023, while a related proteomics study in aged mouse hearts documented that elamipretide treatment attenuated age-associated post-translational protein modifications White et al., GeroScience, 2021. A 2024 preprint extended this work, reporting that elamipretide improved cardiac and skeletal muscle function during aging in mice without detectable changes to epigenetic or transcriptomic age markers Pharaoh et al., bioRxiv, 2024.
In neuroscience-focused research, Cao et al. reported that elamipretide improved mitochondrial dysfunction and reversed synaptic and memory impairment induced by lipopolysaccharide (LPS) challenge in mice, a model used to study neuroinflammation Cao et al., Journal of Neuroinflammation, 2019. More recently, Watson et al. examined SS-31 in both in vitro and in vivo spinal cord injury models, reporting effects on cardiolipin alteration, neuronal survival, and lipidomic profiles Watson et al., Int. J. Mol. Sci., 2025. In metabolic disease-focused cell and animal work, Zhu et al. reported that SS-31 activated the mitochondrial glutathione peroxidase 4 (mitoGPX4) pathway and reduced markers of mitochondria-dependent ferroptosis in a diabetic cardiomyopathy research model Zhu et al., International Journal of Molecular Medicine, 2024.
Proposed Mechanism of Action
The literature converges on a lipid-membrane-based mechanism rather than classical receptor binding. SS-31 partitions into the mitochondrial inner membrane and binds cardiolipin through combined electrostatic and hydrophobic interactions, which is proposed to stabilize cristae architecture, prevent cardiolipin peroxidation by cytochrome c, optimize electron transport chain efficiency, and reduce production of reactive oxygen species Szeto, Br J Pharmacol, 2014. A cross-linking mass spectrometry study further identified SS-31 protein interaction partners among oxidative phosphorylation complexes and 2-oxoglutarate metabolism enzymes, both known cardiolipin-binding protein groups, providing molecular-level support for this membrane-mediated mechanism Chavez et al., bioRxiv, 2019.
